What does the 3-hole design actually change on track?

The extra hole gives a gentler initial damping rate, which can make the car less nervous into corners and improve wheel hook up on corner exit. Expect a smoother first part of the stroke without losing mid and end stroke control.

Is any special tool needed to fit BM-S143A?

No special tools are required beyond your normal shock service kit. Standard shock disassembly, piston removal and careful reassembly with correct shock oil is all that is needed.

Should I change shock oil after swapping pistons?

It is a good idea to re-evaluate oil weight after fitting the 1.4X3 piston. Lighter or heavier oil can be used to tune compression and rebound to match your driving style and track conditions.
